Social Services

There are three basic components to the Social Services Department at Grand View: discharge planning, support, and information and referral.

Discharge planning includes working with the patient, family, and medical staff to develop post-hospital care. This may include referrals to nursing homes, psychiatric or alcohol/drug rehabilitation centers, home health care, or other appropriate community resources.

Social Services’ support is multi-faceted, including facilitating support groups and responding to emergencies.
